Many patients face a critical transition back to everyday life after hospitalization and rehabilitation, often marked by gaps in care, unclear responsibilities, and a lack of continuity. Under the title “Digital Health in Aftercare – From Fragmented Care to a Continuous Care Pathway,” the PICS Symposium 2026 invites participants to address these challenges on April 13, 2026, in Vienna.

Long-term consequences such as physical impairments, cognitive deficits, and psychological burdens often meet a fragmented healthcare system. The aim of the event is to discuss practical solutions and to strengthen collaboration across professions and levels of care. Digital health can help bridge these gaps – but only if it is meaningfully integrated, accepted, and feasible in everyday practice.

A highlight of the morning program is the keynote by Dr. Matthew F. Mart (Vanderbilt University Medical Center, Nashville), who will provide an international perspective on PICS, post-acute trajectories, and the importance of structured follow-up care.

Important: The morning session serves to establish a shared understanding and common baseline. The main interactive component takes place during the afternoon workshop (13:15–16:45). The goal is to prioritize key areas for action and jointly develop consensus-based recommendations for PICS aftercare, including digital elements.

The event is aimed in particular at physicians, intensive care nurses, physiotherapists, psychologists, pharmacists, as well as discharge and case management professionals and others involved in follow-up care.
